SHOR Health

For providers

Family support your organization can offer, without adding to anyone’s job

When a patient leaves your program, support for their family usually stops. SHOR gives the family member a full year of support by text. You hand them a card. SHOR does the rest.

Founding pricing is open to the first ten facilities.

SHOR

Morning

You can love someone and not pick up. If a call comes when you’re not steady, let it ring, then text: “Saw you called. I’ll call you back at 8.” Then call at 8.

Evening

How was today for you, 1 to 4? 1 rough, 2 not great, 3 okay, 4 good.

3

Solidly okay, and okay days do more good than they get credit for. Enjoy a quiet evening.

How it works

Three steps, and your staff only do the first one

  1. 1

    Staff hand out a card

A card with your facility’s name and a code, at the first call, admission, family night, or discharge.

  2. 2

    The family member signs up

On their own phone, in about two minutes. They agree to the texts themselves.

  3. 3

SHOR texts them for a year

A practical text every morning and a check-in every evening.

A year of engagement

The member’s year with SHOR

Twelve months from the day they sign up, including the 90 days after discharge.

  1. Day one

They get a welcome text. Their first morning text comes the next day.

  2. The first weeks

They practice what to say in hard moments, by text.

  3. Discharge

Your program’s contact winds down. SHOR’s texts keep coming.

  4. The first 90 days after discharge

Daily support through the hardest months at home.

  5. Month twelve

A full year of support, offered by your organization.

What the family member gets

The hard conversations get easier when you know what to say

  • Words that work

What to say when they call wanting to leave treatment early, ask for money, or come home. Members practice by text.

  • Two texts a day

A practical idea every morning and a check-in every evening.

  • Help in the moment

Members text SAY to get the right words for what is happening right now.

  • Clinically reviewed

Built on approaches research supports, written by people with direct experience, and reviewed by a clinician.

Waitlist and aftercare

Before the bed opens, and after discharge

The waitlist

Keep families connected until a bed opens

Hand out a code on the first call. The family member gets daily support while they wait and stays connected to your program.

The first 90 days after discharge

Daily contact after yours tapers off

Families feel most alone in the weeks after discharge. SHOR texts them every morning and every evening.

What you see

A monthly roster of who signed up

  • Each month: first name, phone, enrollment date, and whether the account is active
  • A monthly summary of enrollment and engagement, in totals
  • Your name on the card, the enrollment page, and the report
